Perhaps Haku did look like a boy at one point. But Zabuza needed a girl in his life and Haku was the only one available to him as other women feared him because he was the Demon of the Hidden Mist.

No, Haku pretty much has always looked like a girl and has embraced it fully (pink kimono, picking flowers...).
