hollow ichigo came out only cuz ichigo was ..well.. dying (kinda).
hollow ichi came out to rescue the ichigo that was getting owned.
I don't think he was dieing it was more of that ichigo's mind became to weak to keep back the hollow any longer allowing it come out and play so to speak.
